Requesting an Official Copy

Need a full chart or certified record? Complete the "Authorization to Release Medical Information" form, then email, fax, or mail it to our Health Information Management department. Most requests are processed within a few business days and can be delivered by secure email, standard mail, or in-person pickup with photo ID. Sending records to another physician is always free, and our HIM staff is ready at 713-980-7900 to walk you through any special instructions.
